"A Calendar to Live By" 2009

Commemorate birthday's anniversary's, and other special days for all your loved ones, not just those who have been affected by cancer. Days may be purchased for a contribution of $50 each and there is no limit to the number of people you honor.

Please indicate the name of your loved one, whether the recognition is 'in honor of" or "In memory of," and your first, second, and third choice for days. While every effort will be made to grant your 1st choice, only one name will be printed on each day, and days will be given on a first come, first served basis.

All forms and checks must be received by August 15th to ensure inclusion of your loved one in this very special calendar.
